Clarity Evaluations — Privacy Policy

Clarity Evaluations (“Clarity,” “we,” “us,” or “our”) is a service of Woodlands Psychotherapy and Brain Training, operating the website clarityevaluations.org (the “Site”). This Privacy Policy explains what information we collect, how we use it, and the choices you have.
The nature of this service
Clarity provides structured ADHD evaluations based on validated rating scales, reviewed and signed by a licensed clinician. Because you (or your child) receive a clinician-reviewed evaluation, some of the information we collect is health information. Information you provide as part of a purchased evaluation is treated as part of a clinical record and is protected under our Notice of Privacy Practices, which is available on request from our office. This Privacy Policy explains how the Site handles your information; where the Notice of Privacy Practices applies to protected health information, it controls.
Information we collect
Free screener. If you use our free screening questionnaire, we collect your responses, an email address if you provide one, and a severity score. The screener is educational and is not a diagnosis.
Evaluation purchases. When you purchase an evaluation, we collect: your name and contact information; for child evaluations, your child’s name, age, and the information you provide about them; your (or your child’s) responses to the rating scales; optional history and context you provide; and, for the Advanced tier, results of the QbCheck objective test administered by our vendor Qbtech.
Teacher questionnaires. For child evaluations, you may invite a teacher to complete a rating scale. We collect the teacher’s responses and the contact information used to send the invitation.
Payment information. Payments are processed by Stripe. We do not receive or store full card numbers; we receive confirmation of payment and transaction details.
Technical information. Like most websites, we and our service providers collect technical data (IP address, browser and device type, pages viewed) through cookies and similar technologies.
How we use information
We use the information you provide to deliver the service you purchased: scoring your rating scales, preparing and releasing your evaluation documents, communicating with you about your evaluation’s status, and offering the optional follow-up consult. We use technical information to operate, secure, and improve the Site, and payment information to process transactions and maintain business records. We do not sell personal information.
Children’s information
Child evaluations are completed by a parent or legal guardian about their child; the Site is not directed to children, and children should not use the Site themselves. Information about a child submitted by their parent or guardian is used only to prepare that child’s evaluation. By inviting a teacher to complete a questionnaire, you confirm that you are the child’s parent or legal guardian and that you authorize us to contact the teacher for that purpose.
How we share information
We share information only with: service providers who host and operate the Site and process data on our behalf (including Supabase for our database, Stripe for payments, Qbtech for QbCheck test administration, and our email provider); the reviewing clinician and clinical staff who prepare, review, and sign your evaluation; legal authorities when required by law or to protect safety; and successors in a merger, acquisition, or reorganization. Teachers you invite see only the questionnaire — they do not receive your child’s responses or results.
Data retention
Evaluation records are retained under the clinical record-keeping requirements that apply in Texas: records of adult clients are kept for at least seven years after the last date of service, and records of minors are kept at least until the client’s 21st birthday (or seven years, whichever is longer). Screener responses and technical data are retained only as long as needed for the purposes described above.
Security
We use administrative and technical safeguards including encrypted transmission and access controls. No internet service is completely secure; please use the Site’s secure flows rather than email for health information.
Your choices and rights
You may request access to, correction of, or deletion of your personal information by contacting us, subject to our record-keeping obligations for clinical documents. Texas residents may have rights under the Texas Data Privacy and Security Act, including rights to know, correct, and delete personal data.
Marketing communications
If you provide your email through the free screener, we may send you occasional follow-up emails about your screener results and about our evaluation and clinical services. We do not include your screener results or any diagnostic information in email subject lines. Every marketing email includes an unsubscribe link, and you can opt out at any time.
A note about emergencies
Clarity is not an emergency or crisis service. If you or your child are in crisis, call or text 988 (Suicide & Crisis Lifeline) or call 911.
Changes to this policy
We may update this Privacy Policy from time to time. The effective date above shows when it was last revised, and material changes will be posted on this page.
