Clarity Evaluations — Terms of Use

Clarity provides structured ADHD evaluations built on validated rating scales (and, for the Advanced tier, the QbCheck computerized test), reviewed and signed by a licensed psychiatric-mental health nurse practitioner. The deliverable is a signed Clinical & Accommodations Report and a plain-language Family Findings Guide.

The Services are not emergency care, therapy, or ongoing treatment; they are not a medication or prescription service; and the free screener is not a diagnosis. An evaluation describes findings based on the information you provide and standardized instruments — it is not a substitute for in-person clinical care where that is indicated. Purchasing an evaluation establishes a limited, evaluation-only clinical relationship with the reviewing clinician for the purpose of preparing your report; it does not enroll you in ongoing care. Ongoing care, including the optional follow-up consult, is provided under separate intake and consent with our practice.

You must be at least 18 to purchase Services. Adult evaluations are for the purchaser; child evaluations must be purchased and completed by the child’s parent or legal guardian. You agree to provide accurate, complete information — the quality of an evaluation depends on it. Services are available to residents of the State of Texas; by purchasing, you confirm that you (or the child being evaluated) reside in Texas.

Do not use the Site to seek emergency help. Call or text 988, call 911, or go to the nearest emergency room. If information submitted in an evaluation raises an urgent safety concern, we may contact you and provide crisis resources, but the Site is not monitored continuously.

Current pricing is shown at checkout (Standard $199; Advanced $349, which includes the QbCheck objective test; optional follow-up consult $165). Payments are processed by Stripe.

Refund policy. If you have not started any questionnaire, you may request a full refund. If you have started questionnaires but clinician review has not yet begun, you may request a 50% refund. Once clinician review has begun or your documents have been released, purchases are non-refundable. For the Advanced tier, the QbCheck test fee ($79) is non-refundable once your test voucher has been issued, because it is charged to us by the test vendor. To request a refund, contact us at admin@therapythewoodlands.com; approved refunds are returned to the original payment method.

Our standard turnaround is 3–5 business days from completion of all required measures to release of your documents. Turnaround times are commitments we work hard to meet, not guarantees; delays caused by incomplete measures, pending teacher questionnaires, or QbCheck completion pause the clock.

If you invite a teacher or other third party to complete a rating scale, you confirm you have the right to involve them and that they consent to being contacted. Their responses become part of the evaluation record.

The QbCheck test is provided by Qbtech and subject to its own terms and technical requirements (device, camera, environment). We are not responsible for Qbtech service interruptions, though we will work with you to complete testing.

Your signed report and findings guide are provided for your personal, educational, and care-coordination use (including sharing with schools and clinicians of your choice). You may not alter a signed report. We retain evaluation records per our clinical record-keeping obligations.

You agree not to misrepresent identity (including completing an evaluation as someone else), submit information about a child without legal authority to do so, use the Services to obtain documentation by deception, interfere with the Site’s operation, or violate applicable law.

The Site and Services are provided “as is.” We do not warrant that an evaluation will result in any particular finding, diagnosis, accommodation, or outcome — school and employer accommodation decisions are made by those institutions, not by us.

To the fullest extent permitted by law, Woodlands Psychotherapy and Brain Training and its owners, clinicians, and staff are not liable for indirect, incidental, consequential, or punitive damages arising from use of the Site or Services. Nothing in these Terms limits rights you hold under applicable law with respect to clinical services provided to you.

We may revise the Services and these Terms; continued use after posted changes constitutes acceptance. These Terms are governed by Texas law; disputes will be brought in the state or federal courts of Montgomery County, Texas.
